Australian Primary Care Collaborative
The Australian Primary Care Collaborative (APCC) supports and encourages general practice to deliver best-practice primary health care by reducing lifestyle risk factors and improving care for patients with chronic and complex conditions.
The APCC currently focuses on three areas;
- management of diabetes
- secondary prevention of coronary heart disease (CHD), and
- improving access to primary health care services for the whole community.
The APPC achieves this by encouraging shared learning, peer support, training and education for practice managers, reception and clinical staff.
To date, the APCC program improved the health of more than 260,000 Australians living with diabetes and coronary heart disease by promoting timely access to effective care.
The APCC program is offered at various times of the year and GSGPN welcomes expressions of interest to participate in future programs.
